For care & support organisations

A habit,
not a handout.

It starts small. Diagnoses, medications, the questions worth remembering. From then on, every visit begins with "What's in your Care Card this week?" - and nothing slips between them.

The information never leaves the carer's device. Never reaching us, or you.

Why it fits your organisation

You hand out a card - and help them start a habit.

Like being handed a gift card

A carer receives a card and a code from someone who already supports them. They scan, add the essentials, and they're set. Built for people under pressure, not power users.

You support them - and hold none of their data

The care summary lives on the carer's device, not in your systems. Support more people without taking on more risk.

30 seconds to activate. 2 minutes that change every visit.

A carer enters one code and adds their first few details and questions - about two minutes, start to finish. That small habit is the whole point: from then on, your team can open every visit with "What's in your Care Card this week?"

~2 minstart to finish
Support your carers

Give your team a way in at every visit.

A carer's questions don't always surface in the room - they surface at 2am, in the car, or after the worker's gone. The Care Card catches them. When your support worker arrives, there's a real starting point: "What's in your Care Card this week?" - and the visit begins with what actually matters, not with trying to remember.

Remember what matters, when it matters

Carers jot down questions the moment they think of them - ready to raise at the next visit, with whoever they see.

How activation works

Designed to fit how you already support carers.

1

We issue codes to you

As physical wallet cards or a digital batch – whatever suits how you hand them out. No setup on your side.

2

Your team hands them out

At intake, in a welcome pack, or through a peer mentor – however you already connect.

3

The carer activates

One code, one device. The card is theirs, for good.

No accounts to manage. No ongoing cost to the carer.
The data question

What we keep about your carers: nothing.

We know we issued your organisation a batch of activation codes, and when they are activated. That's the whole of it.

We never know who activated a code. We never see what's inside a card – every diagnosis, medication, note, everything is encoded on the carer's own device and never sent to us.

It's the digital version of the paper care summary you already hand out. To read a word of this one, you'd need the carer's phone in your hand, unlocked.

Questions & answers

Common questions

What is The Care Card?
The Care Card is a portable care summary for families and carers. You record the key details about the person you care for - medications, diagnoses, allergies, practitioners and emergency contacts - and carry it as a scannable QR card, or downloadable pdf that you can share with any doctor, nurse or support worker.
Can organisations provide The Care Card to their carers?
Yes. Carer support organisations and care providers buy Activation Codes in bulk and hand them out - at intake, in a welcome pack, or through a peer mentor. Codes arrive in batches ready to distribute, with no portal to run and no per-seat admin. Bulk pricing is available on enquiry, scaled to your volumes, with co-branded options available.
Does our organisation hold any carer data?
No. The care summary lives on the carer's own device, encoded into their QR code or PDF - there's no central database, and nothing is stored on our servers or yours. You never hold carer or patient data, so there are no records to secure and no breach liability sitting with you.
How much does it cost?
$69.95 +GST, for an individual card - yours to keep, with no subscriptions or renewals, and it lasts forever. Organisations buy Activation Codes in bulk to provide to the carers they work with, with pricing scaled to volume on enquiry.
Is there an app, or is it web only?
The Care Card works in any web browser today, with nothing to install. Native iOS and Android apps are available now on the App Store and Google Play. The web and the apps do all the same things - activate, build, share by QR, print and update. The one addition in the apps is Multiple Care Recipients: look after more than one person from the one device, each with their own card.
Can I look after more than one person?
Yes, in the iOS and Android apps (available now). With Multiple Care Recipients you can hold a separate card for each person you care for on the one device and switch between them. The web version is built around a single card.
Where is my data stored?
On your own device. The care summary is encoded into the QR code itself - there's no central database and nothing is stored on our servers. Your information stays with you.
Do I need an account or internet connection?
Only to activate. Enter your code once while online, then build and share the card anywhere - no signal needed. Perfect for remote areas with limited, or no, internet access.
How do I activate my card?
Enter the Activation Code you received and you're ready to go. If you need to access your details on a different device, you can either upload your Care Summary PDF (which we encourage you to download) or scan a QR code.
